We don't recommend this - often the more approvals you add, the further those people are from being able to know if they really should approve - and you will end up with people just approving things because they assume they should.

Please educate your client about the financial and human cost of such a process. 

There are numerous studies concluding it will almost certainly lead to decreased efficiency, reduced employee autonomy, increased frustration, and lower motivation, all of which negatively impact overall performance.
