I run a custom 3D printing business in which I may only ever make one of a given item.
Can MRP be setup in a way that I don't create a "product" for each item? Instead, the "Product" is just 'custom print' but the BOM will be different every time.

It seems like there would be good reason to create a product for every item. It's a way of keeping track of your customer's part. They may want to order a duplicate print. You could associate STL files, or other design data, with the product, using the "Document Management System" module. Maybe you need a customization to automatically generate the product and BOM from the sales order?

Could you clarify your need? You can easily define several BoM that make the same item. Then, when you select this item in a new Manufacturing Order, you will be able to select one of all the BoM you priorly defined. Is that meet your need?
