How to add approval in stages(eg in contract proposal stage) while using the recruitment app?

Dear Ray! Thanks for your response to my query.In the recruitment app, go to the stage "contract proposal". In this stage i cannot able to create approval.However for refuse alone i am able to create approval, which is not required for me. The manager needs to be notified in this stage and need his approval.post which it must go to next stage "contract signed". Please help on this!!
Have you seen:
not fully working...can you send screenshots pls?
Can you define "not fully working"? At which step is it confusing / do you get an error / unexpected result?
