Read the Response to this Question and at Step 2 select your model (the model defined in your custom module) and the related date field.
@Med that looks perfect! Could you also trigger this kind of e-mail action with a non-date field? For example send an automated e-mail on the creation of a lead? Is the concept the same then?
You can use the same solution by selecting Lead/opportunity as Document Model and Creation Date as Trigger date.
